Attached below is the CPCE Medical Form.

This form is to be completed by ALL First Year Students, please ignore if you have done so already. It is one of the required documents to successfully register as a teacher trainee at the Cyril Potter College of Education.

This form is to be printed and carried to a medical practitioner who will complete it. You are however, required to complete section A of the form prior to meeting the medical practitioner.
